CAT Online Registration Form for Institutional Memberships

Institutional membership is $145.00 per member.

Instructions: Please Read!

  • Institutional membership registration opens on May 1st and closes at midnight on October 1st. Institutional membership is for CAT's fiscal year which is from July 1st until the following June 30th.

  • All forms and payment must be received by October 1st of the current fiscal year.

  • Once the form is completed you will get a copy in the email in-box for the primary contact person. You should be able to use this email for Perkins reimbursement.

  • After clicking the submit button you will be directed to a web page that will provide you with payment instructions and options. If this payment page does not open you could have a popup blocker stopping it. If this happens the email you receive will have the web address to the payment web page. Just copy and paste this address into your web browser address field.

  • Before you start you will need the names, home addresses, home or cell phone number and email address for each and every member you wish to register.

  • You should use this form to pay by: Check, Credit Card or PO.
